The Sunmark Foundation sets aside a limited annual budget for charitable contributions and corporate sponsorships. Please use this form to submit your request. Your request must be received by the Foundation no later than 60 days prior to your deadline.

Donation and sponsorship requests are reviewed monthly. We will notify you as soon as possible; however, please allow up to 45 days for us to contact you.
